Why doctor burnout peaks mid-career. Happiness and the ‘arrival fallacy’

This week, I have been reflecting on why doctor burnout levels peak mid-career (11-20 years post qualification) https://lnkd.in/evRgNjg3. While the reasons are complex, I am noticing a consistent theme in my coaching around what Tal Ben-Shahar calls the ๐€๐ซ๐ซ๐ข๐ฏ๐š๐ฅ ๐…๐š๐ฅ๐ฅ๐š๐œ๐ฒ.

This is the false belief that achieving a specific goal will bring lasting happiness.

Psychologists also call this ๐ฉ๐จ๐ฌ๐ญ๐ฉ๐จ๐ง๐ž๐ฆ๐ž๐ง๐ญ, the belief that a future emotional reward will compensate for the demands felt in the present.

So, we are motivated throughout the extraordinary demands of medical school and specialty training by the promise of ๐ฌ๐ฎ๐œ๐œ๐ž๐ฌ๐ฌ ๐š๐ง๐ ๐ก๐š๐ฉ๐ฉ๐ข๐ง๐ž๐ฌ๐ฌ. We define success as attainment of our CCT and believe that happiness will follow.

There are several ๐œ๐จ๐ง๐ฌ๐ž๐ช๐ฎ๐ž๐ง๐œ๐ž๐ฌ:

1. We fail to stay fully present throughout our training. We donโ€™t notice and absorb positive emotions, ๐š๐ฐ๐ž, ๐ ๐ซ๐š๐ญ๐ข๐ญ๐ฎ๐๐ž, ๐ฃ๐จ๐ฒ, ๐ฉ๐ซ๐ข๐๐ž, ๐ข๐ง๐ฌ๐ฉ๐ข๐ซ๐š๐ญ๐ข๐จ๐ง, love, serenity. We ignore them in service to the idea that we will experience them all when we โ€˜arrive.โ€™

2. ๐–๐ž ๐ฌ๐š๐œ๐ซ๐ข๐Ÿ๐ข๐œ๐ž ๐จ๐ฎ๐ซ ๐ง๐จ๐ง-๐ฆ๐ž๐๐ข๐œ๐š๐ฅ ๐ข๐๐ž๐ง๐ญ๐ข๐ญ๐ฒ. Hobbies, friendships, family, holidays, health, creativity are all de-prioritised in service to our dominant doctor identity; the one we believe will ultimately give us happiness.

3. We arrive at our CCT goal but donโ€™t experience the long-term, sustainable happiness we anticipated. ๐ˆ๐ง ๐ญ๐ก๐ž ๐ž๐ฆ๐จ๐ญ๐ข๐จ๐ง๐š๐ฅ ๐ž๐ช๐ฎ๐ข๐ฏ๐š๐ฅ๐ž๐ง๐ญ ๐จ๐Ÿ ๐ก๐จ๐ฆ๐ž๐จ๐ฌ๐ญ๐š๐ฌ๐ข๐ฌ, ๐ฐ๐ž ๐ฆ๐จ๐ฏ๐ž ๐›๐š๐œ๐ค ๐ญ๐จ๐ฐ๐š๐ซ๐๐ฌ ๐ž๐ช๐ฎ๐ข๐ฅ๐ข๐›๐ซ๐ข๐ฎ๐ฆ ๐ช๐ฎ๐ข๐œ๐ค๐ฅ๐ฒ. We ๐Ÿ๐ž๐ž๐ฅ ๐œ๐ก๐ž๐š๐ญ๐ž๐, ๐ž๐ฆ๐ฉ๐ญ๐ฒ, ๐›๐ฎ๐ซ๐ง๐ž๐ ๐จ๐ฎ๐ญ.

4. This profound disappointment often goes unspoken due to our โ€˜๐›๐ž ๐ฌ๐ญ๐ซ๐จ๐ง๐ โ€™ conditioning. We suffer from pluralistic ignorance, the unspoken truth that others are also suffering in their working lives. And, instead, we believe:

But by way of introduction, a ๐ค๐ž๐ฒ ๐ž๐ฅ๐ž๐ฆ๐ž๐ง๐ญ ๐ข๐ฌ ๐ญ๐จ ๐ฎ๐ง๐๐ž๐ซ๐ฌ๐ญ๐š๐ง๐ ๐ญ๐ก๐š๐ญ ๐ญ๐ก๐ž โ€˜๐ก๐š๐ซ๐ ๐ฐ๐จ๐ซ๐ค ๐ฅ๐ž๐š๐๐ฌ ๐ญ๐จ ๐ฌ๐ฎ๐œ๐œ๐ž๐ฌ๐ฌ ๐ฅ๐ž๐š๐๐ฌ ๐ญ๐จ ๐ก๐š๐ฉ๐ฉ๐ข๐ง๐ž๐ฌ๐ฌโ€™ ๐ฆ๐จ๐๐ž๐ฅ ๐ข๐ฌ ๐ญ๐ก๐ž ๐ฐ๐ซ๐จ๐ง๐  ๐ฐ๐š๐ฒ ๐ซ๐จ๐ฎ๐ง๐. It’s actually the precise opposite

In reality, happiness in the present moment leads to both success and hard work. We experience this when we are:

1. ๐ข๐ง๐ญ๐ซ๐ข๐ง๐ฌ๐ข๐œ๐š๐ฅ๐ฅ๐ฒ ๐ฆ๐จ๐ญ๐ข๐ฏ๐š๐ญ๐ž๐ by the experience of ๐ฉ๐จ๐ฌ๐ข๐ญ๐ข๐ฏ๐ž ๐ž๐ฆ๐จ๐ญ๐ข๐จ๐ง๐ฌ

2. deeply experiencing the satisfaction of ๐๐ž๐ฏ๐ž๐ฅ๐จ๐ฉ๐ข๐ง๐  ๐ฆ๐š๐ฌ๐ญ๐ž๐ซ๐ฒ

3. ๐š๐ฅ๐ข๐ ๐ง๐ž๐ ๐ฐ๐ข๐ญ๐ก ๐จ๐ฎ๐ซ ๐ฏ๐š๐ฅ๐ฎ๐ž๐ฌ, ๐ฆ๐ž๐š๐ง๐ข๐ง๐  ๐š๐ง๐ ๐ก๐ข๐ ๐ก๐ž๐ซ ๐ฉ๐ฎ๐ซ๐ฉ๐จ๐ฌ๐ž

This ๐Ÿ๐ฎ๐ง๐๐š๐ฆ๐ž๐ง๐ญ๐š๐ฅ ๐ฌ๐ก๐ข๐Ÿ๐ญ from a focus on happiness in the future to finding happiness in the moment can support us to thrive and find joy in our working lives.
